This questionnaire is designed to help you to find the balance of proteins and carbohydrates which works best for your body.

Before you answer the questions, please take time to keep the diet diary for at least 4 days. Eat as you normally do, and follow the instructions on the diet diary. This will help you to give accurate answers to the questions which follow and give more insight into the right diet for you. Tick the box which best describes you. If none of the answers seem appropriate then leave that question unanswered.

When answering the questions bear in mind the following:

High protein and high fat foods are beef, lamb, pork, chicken, fish. Nuts, seeds and avocados are also high fat foods.

Carbohydrate foods are vegetables, fruit, cereals, bread.

 You should bring your completed diet diary and this questionnaire to the first session of the course.
